A thorn in your side goes on troubling you, picking into you and never lets you sleep in peace. So, that sentence means that the person will always trouble you.

This expression is take from the Bible story of Paul and the thorn in his side.

Paul received a thorn in his side and it was very annoying.
Paul asked that it be removed but God said, no.
He was to learn to live with the little annoyances and move on.

Ever have somebody in your life that aggravates, annoys, and bugs you to the point of complete irritation and frustration? I'm sure you have.
