Read each sentence, and make sure you understand that sentence before going on to the next.

Take notes while you read, where you put what you've just read into your own words. If you can't, read it over again until you can.

If necessary, look up unfamiliar words (if you can't figure out what they mean from the context).

When necessary, go back and reread something that you missed, then you realize you aren't following what's being said.

Spend a lot of time reading things that are just a bit above your current reading level.

Ask for help at school -- a tutor, a Resource Teacher, or whatever assistance your school has.

Keep at it.

My advice would be to just relax and try to delve into the story. Just put yourself into the story with your imagination. If you are reading a story about a boat capsizing, imagine yourself in a boat capsizing. If you are reading a story about a mountain lion attacking a motorist, pretend you are the motorist. Helped me. I have horrible a.d.d. Reading to me anymore is like watching a movie in my head, much more entertaining because anything can happen. Give that a try.
