Look for a local jewelry store as opposed to a mall chain store. You'll have the best chance of finding an on site jeweler that can help you. If they don't have one, their staff may be able to give you some names of local jewelry makers that can help you.

I never heard of a jeweler who makes anything while you wait. It takes too long, and they are generally backed up with repairs and other work and you get your item back in a few days to a few weeks.
Any reputable jewlery store will have a jeweler that can make something for you. When you leave it, you will be given a ticket of some sort just like at the dry cleaner's, and you will turn in the ticket when you pick up your item.
I have had many repairs and items made over the years, with different jewelers, and I have never had a problem. They don't want unhappy customers ruining their reputation any more than any other business does.

Find a private jeweler in your area to handle it for you. It will not be done while you wait in most cases but a private jeweler is much safer than a public one.
Our family has a private jeweler who does all of our work as our family holds a large quantity of lose gems and jewels that have been in the family forever. We have a relationship with this man and his jewelery is of far better quality than you will get from a cookie-cutter jewelry store.
Ask around or look in the phone book.
Good luck!

My father had a peice of amthest he found in North Carolina cut, polished and set for my mother (its her birthstone). However, it took a week. The stone's polishing time really is the kicker. A good jeweler in you area should be able to offer you "insurance" on the peice. I would check with your local chamber of commerce, and the BBB.
Signature Jewelers in Lebanon, TN is where my father had his done.

nothing much can be made while you wait, but there is a place in nashville indiana that makes jewelry like that. however the setting even if only sterling will cost you far more than the stone i imagine, its called grashopper flats.
