Yes, I have and it's scary. I was filling out an application and that's always the first thing you fill out. I skipped it, I just couldn't remember my name., I finished the application and went back to my name...still no recollection...I had to look at my drivers license.
I also forgot where I was going once. I pulled out of my garage, drove about one block and no idea where I was going. I pulled over to give it some thought, the direction told me it could have been one of several places, after a few minutes I gave up and went back home. Til this day I have no idea where I was going....It couldn't have been very important or I would have missed something. But, if things like this were fairly common with me I think a Doctor would be in order. Luckily these are the only two times over a course of a few years.......as far as I can remember!!!!

I have never forgotten my name but, too often I forget how old I am....lol. Honestly, I cannot tell you how many times I have had to ask my hubby.

No but my husband has. He was filling out a check for me a few months back, looked up at me and asked, "How do you spell you last name?" I almost killed him.
