Yes, a friend did at a buffet at a Casino in the town I lived in. She took a bunch of steak home for her dog. I wouldn't have thought to much of it if it had been her left overs, but she purposely got it for her dog. And it wasn't just one piece it was about 3 or 4.

I am so embarrassed to tell this, but my 2nd cousin took me, my mother, and brother out to eat at a seafood restaurant once. They brought us out some lemon for our fish and my cousin also ask for onion. When we got done eating, she wrapped the lemon and onion up in napkins and stuck them in her purse.
I'll be honest, I did take a beer mug once from a pizza place.

I've never caught her, but my grown daughter steals a fork at practically every restaurant she eats at. It's a game for her.
