There are millions of templates/forms online ... you just need to do a search.

Try http://search.microsoft.com/results.aspx?mkt=en-US&setlang=en-US&q=templates .

Figure out what type of form you would like and try searching for it under form and template using Google. Odds are, someone has the form available -- just make SURE that your antivirus software is updated BEFORE you download ANYTHING!!!!! Microsoft related data is seriously at risk for virus problems.

While you have Word open, go to the help section, and search the type of form your looking for like "cd labels"
