parturition(child birth) is obvious which can sometimes be equalled by the secondary dysmennorhoea due to fibroids etc.also cancer produces irretractible pain that cannot be quelled by any drugs.
but pain is good. it is an evolutionary mechanism to avoid noxious stimuli(danger) and is what causes the withdrawal reflexes when we touch something hot or step on a thorn.
in case of some peripheral neuritis like leprosy the lateral spinothalamic tract(which carries the pain and temperature sensation) and the peripheral nerve endings for the same is destroyed. in fact you may have seen that many of the leprosy patients lose the distal parts of their limbs like fingers and have only stubs. this is because they do not realize when they hit against something or get burnt and therefore keep injuring themselves and finally lose their limbs and other body parts.
in india most of the slum colonies have cats because after they have their food if something remains sticking to their fingers the rats come and chew at their fingers and they don't even realise that they are being eaten alive.

Whats a sick question but well.
Pain does not have an absolute defenition and de experience of pain depends on a BIG part on youre brain. Therefore i belive that not physical torture but mental torture is the most painfull. Specialy if u equal pain to suffer. If u thread a loved one of some one it can hurt million times more and has much more impact on a person.
If it comes to physical pain i belive the cold and hot water baths can be the most painfull. The whole body feels like burning (wich is not) Again the mental aspect is big caus the sensation is not right with the reality.

From the God point,labor was made to be painful after Eve ate the apple.It was one of the punishments for her eating it.And the time a woman is pregnant was also made longer.From the scientific point, anything coming out of a hole not big enough for what's passing is gonna hurt! Most women who have given birth,like me,seem to forget the pain as soon as the baby is out.Just seeing our child makes us forget I guess.The amount of pain each female goes through is based on how that woman's body is built and how good they can take pain.Research has found that women who are naturally red headed take pain very well.I'm not sure why this is,but I'm thankful I've got red/brown hair and take pain pretty well.And I really think us women living today have it so much better giving birth than the women of the olden days.

I see that two answerers have already stated my answer. Childbirth. The good news is that it doesn't last forever - only seems like it will at the time.
There are many very painful diseases and disorders, as well as injuries from accidents. Pain is useful to the body to warn of danger, but I sure wouldn't want to wish anyone pain.
My son who is quadriplegic doesn't feel the pain of a burn or a physical injury. Trust me, it's not good.

It is horrible. Not a single anesthesia is able to get rid of that kind of PAINS deep into my brain.......
