Can graduate schools see the grades you transferred in?

Graduate schools want you to send transcripts from all colleges that you attended -- so Yes.

If you did not do well at your first school -- but did very well at your second school, then that is something you can address in your admissions essay.

I had a horrible freshman year of college -- but did very well in the next three years. My overall GPA was a 3.2 -- but I would have graduated with honors had it not been for my first year of college. I addressed this when I applied ot graduate schools -- and got into several ranked programs (including Berkeley, Duke, Cornell and Minnesota).

Yeah, if they have a transcript of your school records. You need a certain GPA to be able to get in to grad school, so you probably have to send an official transcript with your application.
