Mine was sending out an email to 800 people with just a fax with a load of scribbles attached by PDF.
I was supposed to send out the financial statements.
Not good.

Mixing business relationships with friend relationships. It is better to not discuss your personal life with no one at work, not even somebody you trust.
The second biggest was not checking out what the company paid others before accepting a job too low.

When I was 17, I thought a girl I liked at work would be impressed when I put a entire box of half and half in the microwave and watched them explode like popcorn, then walk out of the store.

Turning up on Monday morning. The worst thing is I seem to repeat said mistake Tuesday, Wednesday, Thursday, Friday mornings aswell - Damn you oversized mortgage payments!!!!

completed the sale of someone's house, sent the money to them, then realised i had forgotten to pay off the mortgage- cost the company £125k until they got the money back from the client. ooops! crapped me pants that day!

im a painter and as i was burning off old paint from the windows at an hotel in st andrews, it caught fire and burnt down completely, at the cost of £7,000,000, sorry
