I'd major in Molecular Bio and then minor in either French or Spanish (prefer Spanish as it's more useful in CA). I suppose you're going to a UC, and not Stanford? It's much easier getting into the science prereq classes that med schools want as a MCB major rather than a French or Spanish major. You're gonna be in school long enough. Don't risk extending it unnecessarily cuz you couldn't get into the classes you need cuz of priority being given to science majors.

There isn't a whole lot of difference between any of these. Any will prepare you for medical school admissions. The only red flag about option four is, with two majors, that might leave little time to take the pre-med classes required of students applying to medical school.

Choose the path that interests you the most.

You speak Spanish WELL. Christ, maybe you better work on your English before anything else.
