Is it just pitch a scoreless inning or is it prevent inherited runners from scoring?

Simple. He enters with runners on and none score.

It`s keeping inherited runners from scoring. If every relief pitcher got a hold for a scoreless inning, you could have 3 or 4 holds in a game.

A hold is given if a pitcher enters in a save situation and gets at least one out without having relinquished the lead. He cannot be the winning pitcher or the closing pitcher. He can get a hold even if a subsequent pitcher ends up blowing the save.

If a reliever comes into a game to protect a lead, gets at least one out and leaves without giving up that lead, he gets a hold. But you can't get a save and a hold at the same time.
