All of those things sounds really good! You are on the right track with those things, but we still haven't come across the item that is the problem. Fill in as much detail about your tank as possible and maybe something will jump out as the problem.

Maybe you have changed 100% of the water to give them clean water? Now the tank has to go through the complete nitrogen cycling process all over again which includes toxic spikes in ammonia, nitrite and nitrate. Any one of these chemicals can kill your fish if the concentration is high enough. A filter does not remove these chemicals, only partial water changes will do it.

Removing all of the water also removes the beneficial bacteria that control the spikes in the three toxins. You should only be doing 30% water changes every week and siphoning half of your gravel every month. This will keep your fish healthy and happy.

You may have added a new fish that was sick, also. If that is the case, you need to tell us about the symptoms of the fish just before it dies. Are there any growths on the fish, small white spots or sunken bellies.

I would take a sample of your water to your local pet store to have them analyze the water. Fish feces gives off a lot of amonia into the water and can be toxic. The first thing to do is get you water tested and then go from there.

maybe you don't feed them enough. some fish need to be fed a small amount 2 times aday. Also, check the PH balance in the water. You might want to get some aquariam salt, its just generally good for the fish! Helps with gill fuction and helps fight diseases!

Fish are weird this way, it happens to the best of us. You should do a complete water change, and sterilize everything. Make sure you condition your water too, so it's safe for your fish. Good luck, hope this helps.
