I've never had a toothache, but I've heard of the agony they cause.
I've had migraines so my choice would be a headache.

Toothache. I deal with chronic backpain and headaches I can handle. Toothaches just kill me!

If I have to choose, it would be the tooth ache. But anytime somebody has a pain they're likely to think that it is worse than anything else, where ever it is.
