When I first started working at McDonald's six years and eleven months ago I was nervous as well. You will do paper work, get a tour, watch some videos or something on the computer. You will be introduced to all the people that are working when you go in.
It took me about a month before I stopped being nervous when I went in. I was worried I would screw up and get fired, come in late to work, forget how to do something I learned, or forget my co-workers names. When the first month was over I became more talkative and better at my job.
Next time you go to work just remember you are learning and you are not expected to know everything. It takes time to learn how to be good at your position. Just be your self and do the best you can. The one thing I have learned working at McDonald's is that you are part of a team and need to be excited and happy each time you come into work.

Don't worry. I'm sure you will be fine. Just pay attention, you're going to learn a lot today, and be super-nice to everybody and all the customers. You don't know who you may need to ask a favor from down the line, so just be nice to everybody, it makes a good first impression.
If you want, take a notepad to write down directions on how to do things. Other people may make fun, but when you have everything down pat in a week your manager will be really impressed.
Don't worry too much about what to wear- just make sure your hair is pulled back and you don't have dangling jewelry or fake nails that could fall off.
Hope this helps! Good luck!
EDIT-
Hilary, if you don't have anything nice to say . . .

You'll be just fine. You will spend some time watching a video on the company's principles and then will be put under the supervision of someone who has worked there for a while. You will probably start out doing easy jobs, like operating the deep fryer and cleaning. Just pay attention and be friendly, and you should do far better than most McDonald's employees!

orientation and fun with the fry vat...you'll spend a lot of time looking and not touching.
You're working so that makes you better than some other people who can but won't work.

It won't be bad. They are generally good at training you and helping you the first couple of days. And you can eat all the chicken nuggets you want. :) Or at least we used to.
