Aside from the AAP guidelines, most states have it as a law that babies MUST be rear-facing until they are a year old and 20lbs - whichever comes LAST. (If you have a 19lb 13 month old, you've gotta keep 'em rear-facing until they gain that last pound, likewise for the 22lb 11 month old - make 'em wait until their birthday.)

Call the police non emergency number and ask their advice. Your friend is putting her child in life threatening danger. Even a sudden stop can cause severe spinal trauma in a baby that young.
Also, you can tell her it voids any sort of warrenty or inferred protection to use a carseat in a way inconsistent with its labeling.

Most rear facing car seats are built for babies up to 20lbs or so many inches (Cant think of the exact #). Mabie the babies Dr. reccomended that she put her in a forward facing seat.
But, there really isnt much that u can to unless u want to call it in.

Even as a Child Passenger Safety Technician if she came to me and I checked her seat, provided it wasn't illegal in my state, I would have to do what she requested. We educate, but we cannot do something to the child that the parent is dead set against.

My son was 5 months old and 17 lbs when I put him in a forward facing seat and my daughter was 7 months old and 19lbs. The thing is when a child is screaming at the top of their lungs because they are laying down in their carseat or they can't see their mama/daddy, it can get really irratating really fast. So while you are driving you are distracted by trying to quit your child down. And that a bigger hazard to the child then being secure in a FF seat. So as long as the carseat is installed correctly and teh child is in the carseat properly I do not see a problem with it.

It's not your kid. It's not your car. It's not your life. Just let it go. You can't force her to to what you want her to do, and neither can the rest of us. Accept the fact that not only are you not in control of the situation, but it's really none of your business. You will feel better if you stop worrying about it.
