Scotts Turf Builder PLUS. Make sure there will not be a frost and cut your grass within 24 hrs of application. Wet your lawn with a sprinkler to help fertilizer adhere to grass. Water your grass according to bag.

Should set you back about $15 for a 10000 sq. foot bag.

UK is part of the key here. Your lawn is too damp if moss is forming and not enough sunlight. Running an aerator may help. You may want to spread some sand that will eventually get in the holes from the aeration and help. Make sure it drains.

The weeds? If you have a decent start to your grass I recommend this: fertilize with a good nitrogen fertilizer. Something in the range of 24 2 11 would be good in the Spring. Water of course, but sounds like you've got enough of that. Cut your grass often! This will help the grass spread. Any kind of weed killer will harm shrubs, trees and probably some of the living creatures too. So, let the grass overcome them. Again, I stress cutting regularly is important.

I have the same problem and am into organic gardening methods only. I have yet to find a product that works. By cutting the grass short enough that dandelions don't have a chance to bloom and throw their seeds, overseeding your lawn with grass seeds to drown out weeds, it can help to a degree. If it is really bad, you may have to tear it all out and start fresh with new grass. I personally won't use all the pesticides or lawn services because I have a lot of birds around that eat the worms in the grass and on top of that, I release lady bugs to kill the aphids and a lot of them are in the grass. I would rather have to look out at weeds than know I have disrupted the nature cycle and killed off beneficial bugs.

You do not state the region you are living in, thus, this is tuff.
I can tell you that based on the general descript you are in a spot where it is both a northern exposure and/or a lot of excess moisture. Moss will not survive unless moist and in shade!
Your problem is more soil and shade related than the plants themselves. You will need to modify the soils and perhaps the shade situation before you can grow either turf of a garden. Fear not, steps can be taken to gradually bring back a turf or garden, at a cost of sweat but not a great deal of money.
You can contact me at gjgjobs@yahoo.com. I will need any picture you can send showing the overall site. I also need to know the region you are in to recommend times and materials to use. I'm retired from Landscape Design and love to assist people with their problems. Good Luck.

For moss, your lawn is too damp, so spike your lawn with a fork and fill the holes with sand.
For weeds the most green way is the hardest! You'll have to pull the whole root system out.
