On most systems there is no way to do this. Why would you want to? Telnet is a very insecure protocol.

Well when you launch telnet you simply give it the port number after the hostname.
telnet youhostname.com 7788
where the port you want to reach is 7788 the server has to be listening on that port for telnet for this to work.

You mean going out? Or coming in?

If you mean connecting out then it depends on your operating system. You might try doing
telnet play.sdmud.org 7777

If you mean you want to run a telnet server on a different port than 23, thats abit more complicated.
