It's a slang trem for a mechanic who is enthusiastic about his work. A race car mechanic, for example, could be called a gearhead. It's not only his job, but also his hobby.

Somebody who is into gadgets. A guitarist with lots of pedal and effects would be a gearhead. .

Gear is slang for drugs. A gearhead is someone who is mentally affected by drugs

Not sure, but the last place I worked, engineers were divided into sparkys and wrench heads. That is Electrical Engineers and Mechanical Engineers.
