They can only drink milk with their mouths but in order for a cow to survive just as with all mammals it requires water. So for a cow to get water it has to drink the water anally. In that is why you see that cows are kept separate during water "slumping" (that's the process of inhaling water rectally) it is unsanitary for the other animals to drink from a "slumping" water hole. It is a strange process but it is necessary to the survival of this majestic animal.
