I make quilts and bears out of baby clothes. I use quilter's rulers and a rotary cutter to get good straigfht cuts out of the clothes. The pieces can be all different shapes and sizes, as long as they all have straight edges.
After they're cut, iron them to some nice stiff interfacing. Then start sewing them together using a quarter inch seam. Just keep sewing pieces on until you have a big piece of patchwork fabric.
After that's done, go back through and sew down each seam allowance so the fabric won't be lumpy. (as circled in this pic http://www.whozatvoice.com/seams.JPG )
Then just cut your pillow pieces from your new patchworked fabric.

This is how I would do it:
Make 2 templates out of paper of the size you want your pillow to be. You can use freezer paper, brown paper, or anything. Stitch random pieces of fabric made out of the baby clothes in a crazy quilt pattern that is the size of your template. Leave 1/4 inch of fabric overhanging on all four sides of the templates and cut off the rest. Put the right sides of the two pillow pieces together. Sew them together, but leave about a 6 inch hole on one side. Turn the fabrics inside out and start stuffing the pillow. Hand sew the remaining hole and then you have a pillow.

simply cut out two muslin squares 18 inches on each side, then apply the baby clothes on top, change them around till you like the pattern and then sew them down, then cut the baby clothes off at the edges so you have two squares again, put them face sides together, thats with the baby clothes touching and the plain muslin sides to you, start about 2 inches from the corner on any side, thats two inches before you turn a corner, then sew all sides, finishing two or 4 inches in from the last corner, then turn it right side out, and stuff with polyfill. then hand sew the opening shut. there you are, one original pillow, if you wanted it the size of a bed pillow, simply take a pillowcase and sew the baby clothes to the outside of the pillow case, one side or both sides. then send him the trimmed pillowcase, put binding on the edges or turn all the clothing under along the edgse so nothing unravels.

Simple patchwork would work just cut 4x4 squares sew them together into the desired size sew together and stuff its easy
