It could be either. There is no way to know for sure unless you get a call from HR.

Depends on the company. What normally happens is the local manager does the first round of interviews, they submit the resumes and their notes of recommendation or rejection to H.R. for processing. H.R. will either set up the next interview or contact a person to tell them that they need to go take a physical or drug test or what ever pre-employment things that they do. It's a positive sign though and not a negative one that you are still in the running.

I don't think it's a negative sign at all - HR is usually the department responsible for hiring new individuals. This may be a good sign - Good Luck!

This is a good sign. HR does your back ground check, referances, etc. This usually means that the interviewer wants to hire you.
