In Hold'em the small blind is always after the dealer, then the big blind. So logically it would seem like the dealer (the button) would be the big blind once only two players remain.
But this is NOT the case!
Everything changes when only two players remain. The button is moved in such a way that if a player was just on the big blind he is not on the big blind again.
The person on the button is then the SMALL BLIND and the person without the button is the BIG BLIND. This is to ensure that being on the button still has the advantage that it does when there are multiple players. By making the button the small blind the button has the option to fold, surrendering only a small blind as opposed to a big blind. He is also last to act in all subsequent betting streets.

In heads up, the dealer is the small blind becasue having the button AND being big bland is too much of an advantage.
Normaly (not in heads up that is) the small blind is to the left of the dealer.

The dealer button is the small blind and he is first to act, but when dealing the cards be sure that the button gets the last card. The button always gets the last card.
