I have heard many sources cite that the butterfly ballot was a source of confusion for many, because they were unfamiliar with the type of ballot. But, I seem to recall that the format of ballot was quite common, and had been used in other elections. It was only a first for the presidential election in Florida because of the number of people running for the office in 2000. Can any one point me to a source that definitively says one way or the other?

That type of ballot has been around for many years.
