"A relief pitcher is credited with a hold any time he enters a game in a save situation, records at least one out and leaves the game never having relinquished the lead. A pitcher cannot finish the game and receive credit for a hold, nor can he earn a hold and a save in the same game."

A hold is given when a pitcher who comes in after the starter and his team retains the lead while he's pitching gets credit for a hold. The last pitcher to do this, as long as the game is within three runs, gets credit for a save.

Kullroyce is correct as far as he goes. But the definition of a hold includes a reference to a save situation, and I'd add that definition as well in order to define a hold.
"SAVES FOR RELIEF PITCHERS
10.20
Credit a pitcher with a save when he meets all three of the following conditions:
(1) He is the finishing pitcher in a game won by his club; and
(2) He is not the winning pitcher; and
(3) He qualifies under one of the following conditions:
(a) He enters the game with a lead of no more than three runs and pitches for at least one inning; or
(b) He enters the game, regardless of the count, with the potential tying run either on base, or at bat, or on deck (that is, the potential tying run is either already on base or is one of the first two batsmen he faces); or
(c) He pitches effectively for at least three innings. No more than one save may be credited in each game. "

A relief pitcher stat which he is credited with if he comes into a game in a save situation (a lead of 3 runs or less or the tieing run at the plate or on deck) maintains the teams lead but does not finish the game.

A hold is almost like a save but not closing out the game. say you are up 3-2 in the 8th and lets say kyle farnsworth comes in for the yanks, if he pitches at least 0.1 of an inning without letting the score get tied and someone else comes in he will have a hold (if they win) but the score has to be within 3 just like you have to have if you are to record a save. Hope that helps.
