If you don't care for yourself, despite your mistakes and faults, then you won't know how to care about other people (who will also be imperfect) [For example, the most unkind and proud people I've ever met didn't accept themselves, and therefore thought they were humble.]
Now if you mean "take care of," then I would also say both. If you don't have the understanding and self-control to take care of yourself well, (mentally and physically - keeping yourself healthy and joyful, purposeful in life) then you don't have the requisite understanding and self-control to take of others. [This may be a subconcious reason why girls are more attracted to muscular and healthy guys]
And if you mean, when push comes to shove, who should you put first, I would say others. That is true selfless love, which not only makes the world a better place, but it adds richness to your life, and their life.
