You can't get one out of a cereal box. You are going have to sue for abandonment. You will also have to go to court to protect yourself and your daughter. Without doing this she could show up and probably sue for custody of your daughter.
Laws differ by state but the site below has links to all of the states and has a lot of good info on it. You can even send questions in and have real experts answer it. I am sure that you can also use the site to find either free or very low cost help.

There are free legal advisors in some towns that you might qualify to see for advise. If you belong to a church, ask around , perhaps there is an attorney who would volunteer to help you some, also , some family courts you can file yourself.. for fees.. You can't avoid all sadly.. Did you file a missing persons on her? .. If she left , after 4 years.. she could be deceased.. And, that is a whole different thing.. I deal in possible facts .. realities . 4 years??? lots can happen.. she could have ended up in a hosp. / or had amnesia . It does happen. or, postpartum depression. Especially leaving a new born behind..
This isn't easy, but, you know your child is safe with you..

You can still get a divorce. If you have the address to any of her friends or family it will be easier, if not you will have to publish ads in local newspapers for a limited time.

You already have the answers. You already know what you must do. The only other thing I can suggest to you is that you either go to legal aid and she is they are willing and able to assist you or you go to your local library and obtain the paper work for a very minimal fee 10cents per sheet and file yourself. Sorry to hear about your troubles...It sounds like you are a good guy and I applaud you for raising your daughter...There should be a legal aid group in your city or town that would advise you about this for free or for very little money....seek it out...good luck and continue being a good dad...

Call your local clerk of court and ask for the procedures..
It may involve newspaper notification for some time period..
