It is the schools themselves that decide they want to move up, generally for the purposes of scheduling top schools as away games to get more money into the program (TV or tickets sales). They petition the NCAA and wait for a decision. Usually the facilities and ability to handle an appropriately conducted Division 1 game determine the NCAA's decision.

Money, alumni support, ATTENDANCE, and consistent winning seasons. Not just a 7-4 or 8-3 playoff appearance...we're talking a legitimate string of conference championships and probably a few shots at a national title. Don't get me wrong, you might be able to get away with a jump up in divisions if you strike at the right moment without these prerequisites, but it doesn't happen often.

They have to have a stadium capable of holding so many seats and sell at least 85% of them out. Plus, they have to have the budget to keep the required number of scholarships available for the team each year and etc.
