That's a good question, because most our rocks are pretty young compared to the earth. As minerals are crushed into the earth, they make their way to the mantel, underneath the crust of the earth, and are melted and reformed, and pushed back up. There is a fault line in the atlantic ocean, where the new rocks, as crust, forms, pushing the old crust away. This causes continents to move, and slowly takes the rocks from the ocean floor and moves them to the highest mountains. It has been evident for centuries by explores who often with discover sea shells on the mountain tops. These rocks are always moving.

Usually the earth. Megma comes out through the volcanos and then its rock. Its the next millions / billions of years which determines that type if rock it becomes (preasurisation / weathering etc). Also a small amount of rocks come from space but most burn up through the atmosphere.

it depends on the rock type.
Igneous Rock - through volcanic processes (eg: lava, magma, etc)
Sedimentary Rocks - layers of particles and sediments that have been compressed and forced together to form a solid mass when there is high pressure and temps
Metamorphic Rocks - either of the above that have been altered in some way (usually high temp and pressure) to form a completley new rock.

Rocks come from other rocks.
The earth is constantly at work tearing down and rebuilding itself. "sedimentary" rocks are made up of bits and pieces of larger rocks that have been broken down into sand or smaller size pieces. "igneous" rocks are from rock material that has melted, and will form a new rock upon cooling. "metamorphic" rocks are made from existing rocks that are heated and compressed to make a different form.

Individual rocks are the result of weathering and erosion, causing them to break off from the one big rock that is the crust of the earth. This crust is formed from molten rock under the surface (magma) being extruded onto the surface and then hardening into igneous rock. Igneous rock that is crushed down into layers becomes sedimentary rock, while rock that is forced to undergo extremes of temperature or pressure becomes metamorphic rock. This all goes round in the rock cycle.
