Basically you want to do a "pro se" filing [filing a bankruptcy without an attorney]. I do not recommend filing without an attorney because I have seen too many amateurs make mistakes. However, it is your choice to hire an attorney or try to go it alone.

You have not indicated the particular jurisdiction in which you live, so it makes the answer a little more difficult. The basic forms are about the same, but the procedure may be different within each geographical area.

A good place to start is this website from the Bankruptcy courts: http://www.uscourts.gov/bankruptcycourts/bankruptcybasics.html

Assuming you are filing after 10/17/05, you may want to read the following publication, which is available in PDF format: http://www.uscourts.gov/bankruptcycourts/bankbasics04606.pdf

Bankruptcy forms, click here: http://www.uscourts.gov/bkforms/index.html

You should contact the Clerk of the United States Bankruptch Court in your particular jurisdiction for more information. Note that they are not permitted to give you legal advice, but they can provide forms and such other publications.

You have enough to get started.
