Go to the Secretary of State's online forms sections. Download the notary application and bond. Go to your insurance agent and tell them you need a notary bond (approximately $100) get the form filled out by the agent and mail it back in with your application fee (approx $25). They'll send you out your commission and you get the stamp at any office supply store.

Check the Department of State website. You take a test and pay a fee for your commission and you must renew it every few years.
