I'd consider my opinion on this unreliable, only because I saw it on discovery channel. They found like one fossil, explaining how it's digestive system reuses certain chemicals to breathe fire, and how its bone structure allows flying.
I tried searching for it later, but couldnt find it, so, um, yeah.

Fire Lizards are Salamanders that would crawl from the rocks of fire circles and were thought by primitives to actually live in the fires. The tale, like many, had the main subject inflate into a larger and larger entity through the centuries.

2 - Yes in the fact that people have called them that, based on their beliefs that a salamander (tiny lizard) lives in the lava (fire)
2 - No - because, as stated before this was a thought, not a fact.
2 - Yes - but that is hard to prove, as it considers parallel universe theories / alternate realities.

Yes.
The universe, in it's infinite size, has probably had any creature imaginable in it at some point in time, but on Earth?
Perhaps there are clues in non-mainstream archeology books.

yes its very very possible for fire dragons existed, anythings possible in the universe the same for big foot and the lock nest and even aliens.
