Even doing a WIndows drive format can leave info on it. The best way is to entirely shred the drive, using a program like Darik's Boot and Nuke ( http://dban.sourceforge.net ).

Stick windows disk in drive. Reboot. If you see "Press any key to boot from cd", then it a key.
If windows just restarts again reboot again and enter setup this time (usually by hitting f1 f2 or del key) and look int he bios setting for "Boot" "Boot Sequence" or something like that. Select it to boot from CD first. Save settigns and restart.
You should get the "Press any key" thing, hit a key and windows install will come up.
Follow instructions until you get to the partitions available, delete the partition (you gotta go thru a couple confirm screens). Then choose the unpartitioned space and hit enter to load windows on it. Follow instructions. Have windows cd key handy.

To remove everything, simply format the hard disk drive. But then why would you want to do that? Surely it would make more sense to just remove the Microsoft virus using your anti-virus software? Just run full virus scan.
You can also upgrade to Linux to be virus free:
http://www.ubuntu.com/
The same CD will let you format your drive too.

Format the hard drive, that will remove everything.
Install Linux and select the ext-3 file system, there's be nothing left of Windoze or your virus. Then it won't come back either.

Make sure your antivirus program is up to date. Do a scan in safe mode. This works really good for me.

If you do not have firefox get it now and use it instead of internet explorer so you dont get any more spyware http://securitynewsfromthenet.blogspot.com/
To remove the spyware/trojan/virus run steps 1-2-3
1) If you have not already scanned your comp with spybot http://securitynewsfromthenet.blogspot.com/2007/03/spybot-search-and-destroy-spyware-and.html do that now .Update it (Latest updates 28-3-2007) and then run it so it gets rid of all the spyware and trojans in safe mode
1b) Run AVG Anti-Spyware Free

2) Do an online virus scan to check and remove virus and spyware if you still have viruses on your computer. And let us know if the online virus scan still finds any virus or spyware .
