"Scarface" Al Capone was a godfather in the Mafia; he controlled the city of Chicago. Albeit, Chicago was always a bit of an outcast in those days. New York and some of the other cities viewed it with some distain as the Wild Wild West.

Coming up in Chicago "syndicate", as it was called then, his boss was his uncle, Johnny "Legs" Torrio.

Since Capone was not Sicilian, he didn't equal to the godfathers in New York and would never be Capo di Tutti Capi.

The boss of bosses could only come from one of the five families:

Genovese
Gambino
Bonanno
Lucchese
Colombo

Chicago, New Orleans, Kansas City, LA and other cities were only backwaters for the the Family. Vegas was an open city. Capone was only boss in his own backyard.

He didn't have a boss, at he peak of his career he was in command of his gang until he went down for tax evasion.
