I honestly wouldn't suggest using plastic as a liner. There is rubber liner that is much better suited to what you are suggesting. Using just cement will work, but it also needs to be sealed with a sealant for cement. If you want fish or plants in your pond, then the sealant will have to be something that won't harm the fish or plants. A rubber liner of good quality won't ever require the work that cement will.

It's best to have some sort of liner. Plain old landscapers plastic will do the trick. If not over time your concrete will deteriorate. not to mention you'll be filling up your pond about every couple of days.

The fish pond will probably need a liner. It just Depends.

Yes, however you will need to use a liner or you will need to seal the inside of the box.
