I think that septicemia/ammonia poisoning is the problem here - have you tried/done a water change recently?

Some varieties of goldfish are naturally heavy-bodied, such as pearlscales, orandas, fantails, and other fancy varieties. It could help if we knew if yours was a fancy (usually double tail)or a common (single tail). If the plumpness is equal on both sides of the body, it may just be the natural shape or overfed. If the plumpness isn't the same on both sides, then there's the possibility of a tumor.

This could be a case of bloat, in which case withholding food for about three days could help. You could also try feeding a cooked pea with the skin removed.

Another possibility is dropsy in the early stages. In advanced stages, the scales will stick out from the body.

My suggestion would be to do a water change of 25-30% and withhold food as a first step. Adding a little epsom salt can also help - start with about 1/8 tsp per 5 gallons of water. If there's no improvement in a few days or you notice scales sticking out from the body, treat with an antibiotic. Sounds like septasemia (spelling ?). Check your ammonia. It's internal for sure. Go to www.goldfishconnection.com and order Metro-meds and Medi-gold. Feed the metro meds first for 2 weeks then start immediatly on the medi gold, which can be fed for up to a month. Its a little costly but in my opinion it works better than anything I have ever used for gf. Make sure your water is correct. No ammonia or nitrite. Stable ph, you could be experiencing the dreaded tank crash, if the ph is normally 7.2 and now its 6.4 it's going downhill qucikly. You will have to get the ph back up to where it was before. Going up ph doesn't hurt as bad as going down. It never hurts the fish to get the ph neutral (6.8-7.2) Check your nitrates, if they are over 40 ppm its time to do a big water change.
It may be too late for the fat one. Even if they are fat they should be able to swim. Get your water right, have the medicated food rushed to you. You may still be able to save them. If the fish won't eat you may have to force feed them. Hold their nose slightly above the water and they will open up nice and wide for you, drop a pellet in.
Don't put any meds in your tank. If you get the wrong one it could mean a faster death for your fish. The petstore cannot possible know what is affecting the insides of the fish, nither can I. I just know a broad spectrum medicated food works and does not interfere with the biological system of the tank. Meds will even if they claim on the package that they do not. Don't believe it.

you're good, the fish is somewhat unwell. The situation is declared as dropsy and is led to via all and sundry of various diverse inner infections. in many circumstances as quickly as you will discover the scales protruding that's totally confusing to handle. you could attempt antibiotics alongside with Furanace or Furan-2, yet to be truthful opportunities of curing him are very slender. As a facet word, you do no longer choose for to permit the fish to die interior a similar tank with different fish, the opportunities of spreading the ailment in charge are extremely intense if this occurs. Your ultimate direction of action in all likelihood is to eliminate and humanely euthanize the unwell fish. MM

You may be over-feeding

It might have a tumor

It could have a desease, like a bladder or lung disease

It might have a parasite in it(like tubeworms)

it's NOT PREGNANT!!!! goldfish lay eggs..

I need more info. That's all I have to offer. It would be easier if I knew the type and current size, the eating habbits, and any other sympums.

Does sound like dropsy, internal infection. It sounds like your fish is in bad shape. Marycin-two works well but your fish might be too far gone.
