Exhaust fumes "are" poisonous but what comes out of a modern cars tail pipe is cleaner then what came in in most cities !
The fumes go through a cat and the engine management decides how much air etc to add to the fuel.

If your thinking its bad for you because you've seen suicides in films with the fumes, you've got to bear in mind that it need to be in a concealed place and for a long time.

You just press the button in your car that stops air coming in until you have moved away from the car in front. Most new cars are clean now - it's the tree huggers in their 1960's camper vans that do the polluting!
