Hello, Kat! The actual filing of the return is free unless you pay someone to do it for you (in which case you are really just paying for their services, not for the filing).
A blank 1040X (or any other form) is available for free download from the IRS website. If you print and prepare the amended return yourself, the only cost to file it is the postage. You may, of course, owe additional tax depending on the result of the amended return.

That depends on what you mean by "pay". If you are filing an amended return by yourself there would be no charge. If you are having a CPA/Tax Preparer file an amended return for you, there may be a charge for filing the amended return depending on the reason(s) for filing an amended return. If you forget to give your accountant some information or you never received it and thought your return was complete then you would probably have to pay your accountant for filing the amended return. If it was your accountant's mistake then he/she should not charge you for filing the amended return. You would still be responsible for paying any additional taxes owed as a result of the amended return, and probably also interest. If it was your accountant's mistake you should be able to have him/her pay any penalties.

Did you file your own taxes initially? Did a tax service do them? Did they make a mistake? If you were the one who made the error or omission, then you will be responsible.
I think you can go onto the free-file program through www.irs.gov, and file an amended, which will print out to paper and must be mailed, but I don't know if they charge.

No, there is never a fee to file a return directly with the IRS.
If you have it prepared by a commercial tax preparer you'll have to pay them for their time and effort.

only if you hire someone to do it for you it's about $90+. Otherwise I don't believe their is a fee. You can check when you download your 1040X from the irs.com website.
