It means that whatever happens to USA economy, whether good or bad affects the entire world. USA is the world's biggest consumer therefore the decline or increase in consumption will affect labor, production and raw materials elsewhere. There are a lot of products that they buy in the US that many people in other countries can't. Americans still own most of the cars, recreational vehicles (RV), planes and yatchs. Americans buy new clothes every season. Just remember that there are 4 seasons in a year. Not many countries do the same.

This expression was first heard just before World War II at the end of the Great Depression. I'm not sure, but it could have been Winston Churchill who said it. In any case, slight changes in the US economy can have much larger effects on the economies of other nations, which supply the goods that Americans purchase.
