Rush Limbaugh certainly the most controversial and one of the earliest. I would consider his web site a blog as it is full of his comments and opinions. You must admit he has quite a following and he is always mentioning his web site on the Radio. That kind of exposure and advertising is hard to beat...

Matt Drudge also has a famous blog which got notoriety during the Clinton years

What really sets these apart isn't just web content but the personalities are constantly being mentioned in other media.
Their name recognition is strong...

Edit:

Mixed media (Web, TV, Radio, and somtimes Print) is hard to beat using a single media. So a BLOG has to get noticed and eventually another media type picks up on it giving it wider esposure. (aLa' Matt Drudge)
