Actually, your rabbit probably has a respiratory problem of some sort such as a cold. It's probably not real bad at this point because you are only noticing it when it grooms itself.

There's a couple ways you can tell if it is respiratory or just kind of a grunting. Check the inside of the front paws, if they are wet or matted (hair-gelled looking) those are signs of a cold. Also look to see if you see any nasal discharge (snot) or wetness around the nose. Another thing you'll want to do is listen for where the sound is really coming from. All of the signs I have mentioned so far are signs of a cold or respiratory problem. If you see none of those signs there is one last thing you will want to check for as far as a respiratory problem. Check to see what part of the body the sound is originating from. If the sound is originating from the chest cavity then it is respiratory. If the sound is originating from the nose but you don't see any of the previous signs that I told you about, then your rabbit is just grunting when it licks and is totally fine. You'll tend to hear grunting moreso in males that have large heads.

As a precaution, it wouldn't be a bad idea to have your vet check your rabbit out.

Yes, it is kinda like us. When rabbit have to stretch to clean themselves, it can be hard for them to breathe, causing them to make weird noises. Or either your rabbit has a breathing problem. Maybe you rabbit doesn't want to clean itself, so it make a sound of saddness or the sound of not wanting to or something, like when a kid doesn't want to clean their room, but they know that they have to.
