The one document that changed the world forever was the American Declaration of Independence...Why?
It was the first document ever seriously to question the traditional paradigm of government. Since ancient times the idea of sovereignty was that it flowed from top to bottom.
God (or the gods -- depending on the society) held ultimate sovereign power. God selects the king, and the king is soveriegn on earth in the place of god -- wielding god's soverign power. All laws, all rewards and punishments, all justice, and all life and death flowed from this unquestionionable power.
This led to the divine rights of kings philosophy that dominated the rest of the world. But the declaration stated that sovereignty actually originated from the People. Imagine that...
It not only stated that soverignty was bottom-up, but it said that people have basic rights the government cannot trample, and when it does, it is the right of people to alter or abolish that government as they see fit.
This idea inspired the French Revolution, and has been thrown in the face of dictators around the world ever since. It is the only really new idea to enter the history of politics in over 2000 years.

The Communist Manifesto by Karl Marx (& Fredrich Engles)
Marx's ideas of a utopian communist state inspired tens of millions to believe it to be possible. More importantly it first inspired, then angered, then inspired again, Vladimir I Lenin who changed the idea of economic progression so communism could exist in his Russia. Marx was supposedly at the hearts of Leo Trotsky, Josef Stalin, Nikita Kruschev, Leniod Breznev, Mikhail Gorbachev, Chairman Mao, and even Fidel Castro. The Manifesto played a major role in the Cold War between the United States and the USSR. It led to the Vietnam Conflict, Korean War, and countless other military conflicts across the world. It also inspired many revolutions through Europe and Asia. Without this document, so many of the world events simply would not have happened and the world would be a different place.

The 'Instrument of Surrender', signed on Sept. 2nd, 1945 by the Foreign Minister Mamoru Shigemitsu on behalf of the government of Japan, which established the armistice between the Empire of Japan and the United Kingdom, the United States of America, the Republic of China, the Commonwealth of Australia, the Dominion of Canada, the Union of Soviet Socialist Republics, the Kingdom of the Netherlands, the Dominion of New Zealand, and the French Republic.
This was the formal end of the Pacific War in the Second World War.

No document ever "changed" history. History is events that happened in the past and as we cannot travel to the past, nothing changes history. Jefferson didn't change history, but his writings were important enough to be chronicled and therefore part of recorded history.

Whether you believe it to be true or inspired or the Word of God or not, the Bible is a document that is at the center of most of the world's conflicts of the past two centuries. In addition to religious conflict, more schools, more healthcare systems, hospitals and charitable organizations, are the result of what can be found in the Bible (I'm referring to the Torah, which is the first five books of the Christian Bible, as well)
