This could easily have been based on real people. In Glendale California there was a nurse who was found out and convicted of killing almost two dozen people. The people were elderly, frail, and not in all that good of shape, so when they died no one really suspected anything. Eventually one of the other nurses thought something was suspicious and turned this nurse in. So the one in the book may not have been 'real' but it could be based on a real person.
