A tip I have is to place an X of cellotape over the spot to avoid the bit slipping and scratching the tile . It also holds it in place making for an easier job.

I put a piece of tape on first and then I chip away at the glaze where I am going to drill with a centre punch . Not a great amount but enough to get through the glaze. I use a masonry drill but i am sure a special tile drill would be better. I lubricate the drill with turps substitute and run it fairly fast. If it smokes and gets hot I slow it down a bit. Don't press too hard - a nice gentle pressure. Its easy if you take care

You need to drill fast but do not apply pressure like you would while drilling into wood. Just let the drill do it's thing and apply little pressure as the drill bit works it's way into the ceramic

If it's a glazed tile, use a bit designed for drilling into glass (they're different from a masonry bit). You'll have less change of cracking. Start slow with low pressure.

Slow speed is best, use a spear shaped drill bit for ceramic like this- http://www.ultimatehandyman.co.uk/tiling/drill_tiles.htm

Or if porcelain use a diamond coated hole saw-

Place insulation tape over the bit you are drilling on the tile and use a masonary bit on medium speed works well for me.
