If you run high current devices, the battery is called upon to put out more power. The alternator is called upon to provide more charge, in turn taking motor power to turn it. At highway speed, it may not be a huge difference, but you can feel it, and see it if you have a tachometer, when you turn everything on at idle.

Only very minutely. Since the engine doesn't know that you're draining the electricity (except for the slight additional strain on the alternator), you will never notice a decrease in mileage. The difference may be noticeable if you're running the A/C, but even that is negligible (in most cases).
