Black Sabbath not only invented the dungeons 'n' dragons lyrical mentality which defines "Heavy Metal", they pushed every known Hard Rock singer of the age into their own AS "Heavy Metal Singers". Ronnie James Dio, Ozzy Osbourne, Glenn Hughes, Ian Gillan all were basic hard Blues/Rock singers before Black Sabbath.
Sabbath also defined the entire downtuned sound which predominates the Metal sound today.
Name 50 Greatest Heavy metal albums, Black Sabbath will have at least six on the list.

People who say Led Zeppelin are not far off, but they are a hard rock band rather than a heavy metal one. They did contribute a lot to the creation of heavy metal, though.
Black Sabbath gets the "greatest" title because they were the ones that really took the hard rock sound and downtuned it and made it a bit more doomy sounding, thus creating true heavy metal. From Ozzy-era to Dio-era to Martin-era, their style has changed quite a bit from a psychedelic doom ("stoner" metal) to straight-up heavy metal, yet a lot of their material is very solid.
AC/DC is also not heavy metal, they are hard rock, as well.
