the president runs the country in democratic systems (like the US, latin american countries) with the aid of his ministers and secretaries. He is chosen by voters like the prime minister but he or she embodies the maximum power so to speak (he is controlled by the senate though)
the prime minister runs the country in mostly european countries where they have parliamentary systems (there is a sovereign but the prime minister runs the executive agenda)

Its pretty much an equivalent position in a government. There are differences when you are speaking of specific countries and their leaders roles but there are differences between each country whatever the title is.
