Well, if you're referring to a particular situation rather than a hypothetical one, here's what I think will happen...
...first the media and comedians will use the more formal term "First Gentleman" until they run it into the ground making fun of it...
...then they will eventually (after so many years) just refer to him with his first and last name, like they do now with Laura Bush.
