His official title would be 1st Gentleman, but we know that anytime a Clinton is president, there will be neither a lady nor a gentleman in the Whitehouse.

He would still be referred to as "President Clinton". I don't see why he is allowed to keep the title though. After all, he was impeached.

Former presidents are still, honorably referred to as "President".

So, officially, it would be Mrs President Clinton and Mr. President Clinton.
