This may sound a bit drastic but I have used mould remover meant for shower cleaning on a white bath towel and a white blouse and it worked well; almost instantly. On the bath towel I just sprayed the cleaner straight on as the stains were extensive and I was prepared to throw the towel away if it didn't work. On the blouse I used a cotton wool bud to apply the cleaner to the spots. Of course rinse and wash well. I suppose one would get the same result with bleach or peroxide but I didn't have any in at the time.

Pour undiluted white distilled vinegar into a spray bottle. Vinegar is great at getting rid of mold/mildew, and it also eliminates the bad odor. Spray the vinegar onto the affected areas. Let it sit for several hours, ideally outside in the sun.
Use warm, sudsy water to wash it, and let it dry in a clean, dry place.

In a plastic wastepaper basket Soak 1/4th cup of bleach and1 gallon of hot water, add 1/2 cup of dish washing compound and leave it for 20 minutes and then wash as usual. Check the label and make sure you can use bleach.

yes, soak the shirt in warm to hot water with vinegar and detergent...this works for just about anything
