This is for the NHL and other professional leagues
Highschool is

One 15 minute break between 2nd and 3rd periods

It's periods not quarters. To make sense there should be 4 quarters. In hockey there are 3 20 minute periods of play. Overtime could extend the game if it ends in a tied score. During the regular season a five minute sudden death period is used but in the playoffs the game continues with extra 20 minute periods until one team scores.

They are called Periods. 3 periods, 20 minutes a piece in the NHL. If it is tied after this, then the NHL uses a 5 minute overtime, then a shootout (unless its the playoffs, then its unlimited 20 minute overtimes until someone scores). In International hockey, its a 10 minute overtime, then a shootout.

If it's tied at the end of that (in the NHL, anyway) there is a 5-minute overtime period that is played 4 on 4 instead of 5 on 5 like the rest of the game, then if nobody scores during that the game goes to a shootout. In the playoffs, they just keep playing extra periods of sudden death OT after the third period.
